29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. Throughout facility - On or about December 27, 2016, the employer had not performed periodic inspections that identified the deficiencies of group lockout / tagout and / or to ensure that documented lockout / tagout procedures were being followed by all authorized employees. Employees engaged in cleaning machinery were thereby exposed to the hazards associated with the unexpected reenergization of equipment. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. Throughout Facility - On or about December 27, 2016, employees were required to clean and sanitize machinery in the performance of their duties. Personal lockout / tagout devices were not attached to a group lockout device when two or more employees were cleaning the shell washers and other operating equipment, thereby exposing employees to the hazards associated with the unexpected reenergization of equipment. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

On or about December 27, 2016, employees were required to work with machinery and chemicals, in a general industry setting. The employer failed to assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). Employees were thereby potentially exposed to hazards in the work place. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(c).
