29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a)  At 7790 Spanish Fort Blvd., Spanish Fort, Alabama:  On or about December 28, 2016 and a times prior; the employer allowed his employees to use the support ladders of the ladder jack scaffold to gain access to the work platform exposing them to a fall hazard of 15 feet 6 inches to the ground below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:  a)  At 7790 Spanish Fort Blvd., Spanish Fort, Alabama:  On or about December 28, 2016 and at times prior; the employer allowed his employee to work from a ladder jack scaffold without fall protection exposing him to a fall hazard of 15 feet 6 inches to the ground below.
