29 CFR 1910.26(c)(3)(i): Where portable ladders were used for access to an upper landing surface, the ladder was not set up so that the distance from the vertical wall was equal to one-fourth the working length of the ladder: a) Worksite: An employee was accessing an approximately 16 ft. high cooler box; the ladder in use was placed at a ratio of 6 ft. to 16 ft.; on or about 12/18/16. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
