29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) The employer did not ensure that four (4) extension ladders (two placed on either side of the house) utilized by employees were placed with three (3) feet of their side rails above the upper landing to assist employees mounting and dismounting the ladder and the top of the ladders were not secured exposing employees to a fall hazard of approximately nineteen (19) feet to the ground below. b) The employer did not ensure that four an extension ladder; placed at the back of the house, utilized by employees was placed with three (3) feet of its side rails above the upper landing to assist employees mounting and dismounting the ladder and the top of the ladder was not secured exposing employees to a fall hazard of approximately ten (10) feet to the ground below.
