29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Country Inn and Suites roof: On or about December 29, 2016 and at times prior, employees were exposed to a fall  hazard of approximately 37.5 feet while installing flashing on a 8:12 pitch roof.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a) Country Inn and Suites roof: On or about December 29, 2016 and at times prior, employees were exposed to catapult hazards while accessing and egressing the roof from the Genie Aerial Boom Lift (serial #10266058).
