29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction:  Location: at the loading dock bays On or about 12/23/16, floor holes approximately 12 inches by 4 inches on either side of dock boards used at the loading dock were not covered while employees were loading/unloading trucks.
