29 CFR 1910.28(b)(8)(i): Repair pits, service pits, and assembly pits less than 10 feet in depth. The employer limits access within 6 feet (1.8 m) of the edge of the pit to authorized employees trained in accordance with � 1910.30.  On or about December, 2016, mechanics were allowed to work closer than 6-feet from the edge of the repair and service pit without any fall protection systems such as but not limited to guardrails and warning lines.  This condition exposed employees to fall hazards greater than 4-feet.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields.  On or about December 2016, the employer did not protect employees working around the pit no. 1 Line A from welding light radiation.  This condition exposed employees to UV radiation hazards.
