Envirocraft Corporation, 115 Willard St. Leominster, MA 01453: On or about 1/4/2017, an employee wore a half face-piece, 3M respirator, Series 6001, on a voluntary use basis while painting fabrications. The employer did not ensure the employee was medically able to wear a respirator, and that the respirator was cleaned, stored, and maintained, so the respirator did not present a health hazard to the user.

Employees also use various sealants, solvents and paints including, but not limited to STEEL-IT (SINCO ALKYD PRIMER) and STEEL-IT (Polyurethane), Rust- Oleum, Loctite 242 MS, Loctite 271 HS TL, Klean-Strip VM&P Naphtha Solvent, and Klean-Strip Denatured Alcohol.
