29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: On or about 01/04/2017 at the Hartford Hospital Urgent Care Project, an employee was accessing the roof deck from the ladder access and was stepping down from the top of the parapet wall onto the roof deck and up from the roof deck onto the top of the parapet wall. The distance was measured to be about 27 inches to the deck.
