29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used:  On or about January 5, 2017, employees were exposed to a fall hazard, while accessing a scaffold platform without a ladder.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  a)  On or about January 5, 2017, employees were exposed to a fall hazard greater than 6 feet to the ground level below, while installing drywall on a fourth floor balcony without fall protection.  b)  On or about January 5, 2017, employees were exposed to a fall hazard greater than 6 feet to the ground level below, while moving material for drywall installation without fall protection.  c)  On or about January 5, 2017, employees were exposed to a fall hazard greater than 6 feet to the ground level below, while setting up a one-tier scaffold on a fourth floor balcony without fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about January 5, 2017, employees were exposed to fall hazards, while installing drywall on the ceiling of a fourth floor balcony, without being trained to recognize the hazards of falls.
