(a) Pallet Prep Department - On or about January 5, 2017 employee was reaching inside rear of machine guard to remove cut off material from pallet while the machine saw blade was still in operation.

(a) Throughout the warehouse - On or about January 5, 2017 employees were operating forklift trucks moving pallets to and from work stations at a high rate of speed while employees were working in the same area exposed to being struck by the fork lift trucks.

29 CFR 1910.213(s)(5): Sharpening or tensioning of saw blades or cutters shall be done only by persons of demonstrated skill in this kind of work. (a) North East corner of warehouse by paint booth - On or about January 5, 2017 an employee was operating the Block Pallet Prep machine with the tension on the blade set at 2300psi. That is 300psi over the required max of 2000psi. per operators manual (pg.8).
