29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about 01/09/2017 at the Connecticut Science Center Construction Project, employees were using personal fall arrest systems while working from an aerial lift and the personal fall arrest systems allowed contact with lower levels.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  On or about 01/09/2017 at the Connecticut Science Center Construction Project, the employees were not trained on the use and limitations of the personal fall arrest systems in use
