29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about 1/10/17, on the roof of building under renovation at 226 William St., Buffalo, NY: Two employees were exposed to a potential fall of approx. 12 ft. while installing plywood sheathing on the west section of the roof. No fall protection system was utilized to protect the employees. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 1/10/17, on the roof of building under renovation at 226 William St., Buffalo, NY: Two employees using an extension ladder to access and exit the roof were exposed to a potential fall of up to 12 ft. to the ground because the siderails of the extension ladder only extended approx. 12 inches above the edge of the roof. NO ABATEMENT CERTIFICATION REQUIRED
