29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about January 12, 2017, at the above addressed site, employees performing painting prep work were exposed to a fall hazard of approximately 8 feet when working from the basket of an aerial lift while not wearing a harness with a lanyard attached to the boom or basket of that lift.

29 CFR 1926.453(b)(2)(ix): Controls for the articulating and extensible boom platforms were not clearly marked to identify their function:  On or about January 12, 2017, at the above addressed site, employees performing painting prep work were exposed to a fall hazard of approximately 8 feet when working from the basket of an aerial lift while the controls of that lift were covered with paint and not clearly marked as to their function.
