29 CFR 1926.451(b)(3): Where guardrails were not installed along the front edge of the platform or all employees were not protected from falling with the use of personal fall arrest systems, the front edge of the scaffold platform was more than 14 inches: At 1314 Witherspoon Dr. Prattville, Alabama: On or about January 12, 2017 and at times prior; the employer allowed his employee to work from a ladder jack scaffold platform which was approximately 2 feet from the work area without fall protection exposing him to a fall hazard of 14 feet 6 inches to the ground below.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: At 1314 Witherspoon Dr. Prattville, Alabama: On or about January 12, 2017 and at times prior; the employer allowed his employee to exit the ladder jack scaffold platform by climbing into a window and failed to proved a ladder exposing the employee to a fall hazard of 14 feet 6 inches to the ground below.
