29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    On or about January 12, 2017:    a) Employee was exposed to fall hazards of approximately 9 feet, while riding the forks of a powered industrial truck to reach the working area. When the employer failed to perform frequent and regular inspections of the worksite.       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  On or about January 12, 2017:  a) An employee utilizing a DeWalt 41/2 inch grinder to preform cutting operations, was exposed to struck by hazards, when the employer failed to ensure that guard was installed prior to the use.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
