29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:   On October 19, 2016, in the forge shop, employees operating number 3585 Erie 20,000 pound steam powered hammer press were not adequately protected against a metal piece ejected from the forging area of the hammer during the forming process of a metal part.  NOTE: The employer may opt to utilize feasible engineering and/or administrative controls in lieu of ppe to ensure adequate protection.
