29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Building #5 roof;  On or about December 1, 2016 and at times prior thereto, the Controlling employer, Doug Hollyhand Realty, Inc., failed to ensure that its subcontractor, R. Garcia Construction, protected its employees from fall hazards of 41 feet 7 1/2 inches to the ground below while employees were installing trusses at the roof of the 3rd floor without using fall protection.  b)  Building #5 2nd floor deck;  On or about December 1, 2016 and at times prior thereto, the Controlling employer, Doug Hollyhand Realty, Inc., failed to ensure that its subcontractor, R. Garcia Construction, protected its employees from fall hazards of 30 feet 10 inches to the ground below while employees were preparing trim for trusses without using fall protection.  c)  Building #4 top of wall;  On or about December 1, 2016 and at times prior thereto, the Controlling employer, Doug Hollyhand Realty, Inc., failed to ensure that its subcontractor, Daniel Hernandez dba Daniel Hernandez, protected its employees from fall hazards of 10 feet 3 inches to the ground below while employees were engaged in framing activities without using fall protection.  d)  Building #2 roof;  On or about December 1, 2016 and at times prior thereto, the Controlling employer, Doug Hollyhand Realty, Inc., failed to ensure that its subcontractor, Crimson Contractors, Inc., protected its employees from fall hazards of 30 feet to the ground below while employees were engaged in roofing activities without using fall protection.
