29 CFR 1910.219(b)(1): Flywheel(s) with parts seven feet or less above floor(s) or platform(s) were not guarded in accordance with the requirements specified in 29 CFR 1910.219(b)(1)(i) through (b)(1)(iv):                                                          a) Forge Shop, 200 Martha Street, Blairsville, PA 15717:  Observed on or about January 6, 2017, the flywheel on a Ajax brand upset forge was not fully guarded.  A portion of the flywheel facing the press was not guarded.   Employees operating the upset forge were exposed to caught-by hazards.

29 CFR 1910.305(g)(1)(iv)(D):  Flexible cords and/or cables were attached to building surfaces:   a) Forge Shop, 200 Martha Street, Blairsville, PA 15717:  Observed on or about January 6, 2017, a flexible cord (110 V0lt) used to provide power to a Dayton brand pump was attached to building surfaces.  The flexible cord was attached to the wall above a doorway leading to the Forge Shop.
