29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(I) through (v)), on all open sides:    On or about January 06, 2017, the employer did not ensure that employees troubleshooting the sorting line machine attachments such as the suspended conveyor above the fines transition conveyor and large ferrous materials magnet, were protected from falls. Employees were elevated approximately 25-30 feet above the ground.        Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group:   On or about January 06, 2017, the employer did not establish a procedure to ensure that several employees simultaneously troubleshooting the sorting line were each protected by use of a personal lockout device or equivalent protection. Employees were exposed to amputations from unexpected startup of moving and rotating components while troubleshooting machine attachments, including the suspended conveyor above the fines transition conveyor and large ferrous materials magnet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  On or about January 06, 2017 the employer did not ensure that each employee troubleshooting the sorting line machine attachments affixed their locking devices prior to servicing the machine. Employees were exposed to amputations from unexpected startup of moving and rotating components while troubleshooting machine attachments, including the suspended conveyor above the fines transition conveyor and large ferrous materials magnet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).
