29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:  A work related incident which resulted in an employee hospitalization was not reported to OSHA within 24 hours.  The incident occurred on or about 01/11/2017 at 02:40 pm and was reported on OSHA on 01/13/2017 at 12:55 pm.

29 CFR 1910.178(l)(6): At this establishment, the employer did not certify that each operator has been trained and evaluated to operate the powered industrial truck as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  The employer did not ensure that the required certifications had been completed.
