Kellar and Son Pallet Company: On or about 01/18/17, the employer did not implement a continuing, effective hearing conservation program for an employee (bandsaw operator), who was exposed to noise at an eight-hour time weighted average of 94.5 dBA which is 111.1% greater than the action level of 85 dBA during the 301 minute sampling period on 02/22/17. b.) Kellar and Son Pallet Company: On or about 01/18/17, the employer did not implement a continuing, effective hearing conservation program for an employee (bandsaw operator), who was exposed to noise at an eight-hour time weighted average of 93.9 dBA which is 110.5% greater than the action level of 85 dBA during the 306 minute sampling period on 02/22/17.

29 CFR 1910.95(i)(2)(i): The employer did not ensure that hearing protectors are worn by an employee who is required by 29 CFR 1910.95(b)(1) to wear personal protective equipment: a.) Hearing protection was not worn by an employee (Bandsaw Operator), exposed to noise at 104.4% of the permissible daily noise exposure (eight-hour time weighted average of 90 dBA) during the 301 minute sampling period on 02/22/17. For a full eight hour shift, this exposure would be equivalent to an average sound level of 94.0 dBA. Exposure calculations include a zero increment for the 179 minutes not sampled. b.) Hearing protection was not worn by an employee (Bandsaw Operator), exposed to noise at 103.8% of the permissible daily noise exposure (eight-hour time weighted average of 90 dBA) during the 306 minute sampling period on 02/22/17. For a full eight hour shift, this exposure would be equivalent to an average sound level of 93.5 dBA. Exposure calculations include a zero increment for the 174 minutes not sampled.
