29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:    Location: Powder Ridge Ski Resort, 99 Powder Hill Road, Middlefield, CT 06455  The written Certification of the Workplace Hazard Assessment for Personal Protective Equipment (PPE) was not completed with the following.    1. The person(s) certifying the evaluation.   2. The date(s) of the hazard assessment completed.   3. All job tasks including but not limited the maintenance department and painting operations.
