29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a)     On or about January 17, 2016, and at times prior, the employer, Dubworks SC, LLC, did not ensure that a written respiratory protection program was established and provisions implemented for employees, who are required to use 3M half-face respirators while engaged in spray finishing.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a)     On or about January 17, 2016, and at times prior, the employer, Dubworks SC, LLC, failed to develop and implement a hazard communication program and therefore did not ensure that employees, who are required to spray hazardous chemicals, were protected from chemical hazards.  Employees regularly contact hazardous chemicals including, but not limited to, acetone, toluene, xylene, & n-hexane.
