29 CFR 1910.333(b)(2)(ii)(B): Control circuit devices, such as push buttons, selector switches, and interlocks, were used as the sole means for de-energizing circuits and equipment:    At the substation: On or about January 19, 2017, the employer relied on the use of a selector switch as thee sole means for de-energizing VISTA distribution switchgear.  The VISTA distribution switchgear was de-energized on the out-going side, but remained energized on the in-coming side.Employees were exposed to electrical hazards.
