29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition:    On or about January 17, 2017:    a) An employee was exposed to cave in hazards while attempting to recover a hydra-vac truck from a sinkhole. The employer failed to inspect the excavation, adjacent areas before the employee entered that sink hole.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
