29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    On or about January 16, 2017, at a building renovation site, an employee was allowed to working in the basket of an aerial lift and conduct overhead work without protective head gear, exposing the employee to the hazard of being struck by loose or falling objects.

29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.    On or about January 16, 2017, at a building renovation site, an employee was allowed to work in the basket of an aerial lift without fall protection, exposing the employee to the hazard of falls.
