29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.    VA Salisbury, Bldg #2:  The employer did not ensure that employees working on a scaffold had been trained by a competent person to recognize hazards associated with the work, such as, mounting concrete blocks on top of the scaffold vertical frame.  Employees were also at risk of falling object hazards.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   Salisbury VA, Bldg #2:  On or about January 18, 2017, an employee was exposed to slip hazards created by improperly using a 6 foot step ladder as a straight ladder.
