On or about January 6, 2017, at 5941 South Noland, Kansas City, Missouri 64136: Employees engaged in snow removal activities on a greenhouse roof were exposed to fall hazards of approximately 20 feet.  Floor holes on the roof, on which employees were walking, were not protected to prevent falls to a lower level. The employer failed to provide fall protection exposing employees to fall hazards during snow removal activities. An employee fell through a hole of the green house roof and was hospitalized with severe head injuries.
