29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval:Note:  If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals should have been changed accordingly.  On or about January 24, 2017, and in times prior thereto, a job made man basket was used to lift materials and personnel by a JLG forklift, located at the construction site.  This condition exposed workers to a fall from elevation hazard of 6 feet in the event the man basket upset the center of gravity of the forklift.
