29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  (a) Location: 30 Carroll Circle, Weston, MA 02493. On or about December 1, 2016, an employee was not protected from falling while working/walking in close proximity to unprotected skylights. Skylights were greater than 6 feet above lower level.

(a) Location: 30 Carroll Circle, Weston, MA. On or about December, 1, 2016, an employee was not protected from falling while working/walking on a residential roof at a height greater than 23 feet above lower level.

(a) Location: 30 Carroll Circle, Weston, MA. On or about December 1, 2016, an employee was not protected from falling while using ladders that did not extend at least 3 feet above upper landing surfaces.
