29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)    At the site, on January 19, 2017, the employer did not ensure employees who was performing work on a roof were protected from falls through the use of a personal arrest fall system such as, but not limited to a harness and lanyard system, exposing the employee to the hazards associated with fall hazards that measures twelve feet from the ground level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At the site, on January 19, 2017, the employer did not ensure an extension ladder used to access the roof of a commercial building, where roofing work was being performed, extended three feet above the upper landing surface, exposing employees to fall hazards.
