29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  At this work place, employees conducted welding operations from mobile scaffolds at heights greater than ten feet, and the scaffolds were not fully planked between the uprights and the guardrail supports.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.  At this work place, employees conducted welding operations from mobile scaffolds at heights greater than ten feet without a fall protection system in place.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner.  At this work place, employees conducted welding operations from mobile scaffolds at heights greater than ten feet, and all caster wheels were not locked to prevent movement of the scaffolds. This exposed employees to fall hazards.

At this work place, employees conducted welding operations and utilized welding cables with repairs less than ten feet from the electrode holder.
