29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 25, 2017, and times prior thereto, a. In Line 1A where employees were exposed to struck-by hazards when operating a Dainichi CNC Lathe, Model F-50, without a functioning interlock switch. b. In Line 2A where employees were exposed to struck-by hazards when operating a Mori Seiki CNC Lathe, Model SL-80, without a functioning interlock switch. c. In Line 7A where employees were exposed to struck-by hazards when operating a Mori Seiki CNC Lathe, Model SL-603, without a functioning interlock switch. d. On the east wall of Saw Line 3A where employees were exposed to struck-by hazards when operating a W.F. Well & Sons, Inc. Band Saw without a guard on the unused section of the rotating blade.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. On or about January 25, 2017, and times prior thereto, on the NE corner of the Maintenance Shop where employees were exposed to a fire hazard when exchanging Oxygen gas cylinders that were not separated from fuel-gas cylinders.
