29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a. On or about January 25, 2017, employees using a Porter cable saw to cut wood were not wearing eye protection, exposing themselves to being struck by flying debris.

On or about January 25, 2017, roof area, two employees installing sheathing on a 5:12 pitch roof were not protected from up to a 30 feet fall hazard by the use of a fall protection system.

On or about January 25, 2017, employees used a Husky A-frame 12-foot step ladder in the closed position to access and exit from the roof area.  Employees were being exposed to a 12 feet fall hazard.
