Located at 10799 State Route 104 in Lockbourne, Ohio: On or about October 31, 2016 and at times prior, employees performing work in the fertilizer room were exposed to an electrical shock hazard as a result of a spliced electrical cable which employees used to plug into a 120 volt receptacle to energize an overhead fluorescent light fixture.      b. Located at 10799 Stated Route 104 in Lockbourne, Ohio: On or about September 31, 2016, employees accessing the overhead storage shelf above the blade sharpener in the maintenance shop to access stored tires were exposed to an electrical shock hazard as a result of a spliced electrical cable used to energize the fluorescent light fixture hanging below.
