At Dock #5, the employer had a dock lock system but it was not totally functional with the lighted informational system nonfunctional.  Various forklift operators would enter and exit the trailer without the signal to know whether it was safe to enter or exit the trailer.

29 CFR 1910.178(l)(3)(ii)(A): Powered industrial truck operators did not receive initial training in surface conditions where the vehicle would be operated that could affect safe operation:   On 01/30/2017, Ebro North America dba Riviana Foods located at 611 E. Marceau Street, St. Louis, MO, the employers Dock training PowerPoint program had only one slide addressing Docks and it stated: *Inspect the dock plate,* Check the trailer floor condition, *Trailer wheels chocked, *Nose of the trailer is supported by the tractor or a fixed jack.  The employer had a dock lock system but it was not totally functional with the lighted informational system out.  This dock lock system at Dock #5 was not functioning in the manner to let the various forklift operators know it was safe to enter or exit the trailer. No other procedure was implemented and trained to let the forklift operators know what to do when this condition occurred.
