29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):.    a) 1068 Route 206 South, Bordentown NJ - On or about 01/28/17, two company workers were exposed to caught in and struck by hazards when riding on an unsecured, wooden pallet that was sitting on the elevated forks of a moving JLG Model Number 6042, Serial Number 0160061355 All-terrain fork truck.  At the time of the worksite inspection the operator of this All-terrain fork truck had not received training nor were they evaluated to do so as required by the standard.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
