29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: (a) This violation occurred on or about January 31, 2017 in the east side of the jobsite where employees were working from a scaffold platform that had visible defects in the wooden planks and had not been inspected before the work shift by a competent person exposing employees to a fall hazard of approximately 54 feet to the ground below.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary: (a) This violation occurred on or about January 31, 2017 in the east side of the jobsite where employees were working from a scaffold platform that had gaps larger than 1 inch between adjacent planks and employees were exposed to a trip and fall hazard.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: (a)This violation occurred on or about January 31, 2017 in the east side of the jobsite where employees were walking on 16 feet platforms that were not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping. Employees were exposed to a fall hazard of approximately 54 feet to the ground below.
