29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section:  At the jobsite located at 3614 N Beach Club Circle, Wichita, KS:  Employee engaged in residential framing activities at the edge of the roof was exposed to fall hazards of approximately 13 feet from the eave of the residence to the ground below in that a fall protection system was not utilized.

29 CFR 1926.1053(b)(1): Where portable ladders are used for access to an upper landing surface, the ladder side rails could but did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  At the jobsite located at 3614 N Beach Club Circle, Wichita, Kansas: Employee engaged in framing activities was exposed to fall hazards in that the ladder was not extended 3 feet beyond the access point.
