29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: Production area: The employer is exposing employees to hazardous conditions where the required annual inspections of lockout procedures were not conducted. The absence of required annual inspection of lock-out procedures exposed employees to hazards such as, but not limited to, moving parts of machines with multiple energy sources such as, Flattener, Top Conveyor, Line 200, Building 7 (CV206) due to unexpected activation of residual energy. The lockout procedures were last inspected in the year 2014.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): Establishment: The employer is exposing employees to struck-by hazards where forklifts with defects were not removed from service until the defects were corrected. On and about February 2, 2017, a Toyota forklift (F-10) with missing brake pedal slip resistant cover was not removed from service until the defect was corrected. This defect exposed employees to struck-by hazards from unintended activation of the gas pedal due to operator's foot slipping off the brake pedal.
