29 CFR 1910.303(c)(3)(i): Conductors were not spliced or joined with splicing devices identified for the use or by brazing, welding, or soldering with a fusible metal or alloy. Soldered splices were not first spliced or joined to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose: On or about February 1, 2017 employees were exposed to burns and smoke inhalation because the female end of the extension cord used to power the television in room 229 had been cut off and the bare conductors of the extension cord were connected directly to the blades of the television plug.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On or about February 1, 2017 employees entering the pump room for the spa were exposed to accidental contact with live electrical conductors because an electrical outlet and the light switch box were damaged and were not guarded with an approved cover.
