29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: (a) Servtech at 333 Centennial Pkwy. Louisville, CO: On or before 1/19/2017, the employer had not developed specific procedures to ensure that the hazardous energy associated with the Drill-Line CNC hole cutting machine (Ocean Machinery, Model: MDL-1000/1C, Custom Avenger 40, SN: A002943), including, but not limited to, electrical and hydraulic energy, was isolated and rendered inoperative. The employer did not develop specific lockout tagout procedures for this machine when changing out drill bits and when performing maintenance. Employees were exposed to amputation hazards.

Louisville, CO: On or before 1/19/2017, the employer had not conducted periodic inspections of all energy control procedures including but not limited to the Drill-Line CNC hole cutting machine (Ocean Machinery, Model: MDL-1000/1C, Custom Avenger 40, SN: A002943), to ensure all energy sources capable of causing injury are isolated and rendered inoperative prior to servicing and or maintenance. This condition exposed employees to amputation hazards.
