29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:    1) Jobsite: The employer did not have a written hazardous communications program. Employees were tasked to seal holes on top of a roofs surface, and spackle cracks on a ceiling thus exposing them to hazardous chemicals such as but not limited to:    - Limestone, Hydrotreated light (petroleum), Butan-2-one O,O'O"-(methysilylidyne) trioxime, Aminopropyltriethoxysilane and Octamethylcyclotetrasiloxane contained in "Gecoroofing"  - Limestone, Mica and Crystalline Silica contained in USG sheetrock Easy Sand 20  etc  Noted on or about 1 FEB 2017.

One of the ladder feet was stuck in the upward position preventing the ladder from being set on a firm foundation. The employer did not ensure that a competent person inspected the ladder for visible defects exposing employees to fall hazards. Noted on or around 1 FEB 2017.
