Worksite at 2500 W 81st St. Tulsa, OK 74132. On or about February 08, 2017, employees were exposed to struck-by hazards from excavating equipment and spoil pile not being set back at least two (2) feet from the excavation edge.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:     Worksite at 2500 W 81st St. Tulsa, OK 74132. On or about February 08, 2017, no protective devices were in place to protect employees while working in a trench at a depth greater than five (5) feet.  Employees were exposed to the hazards of being crushed by falling soil.
