29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:  a)  Warehouse by trash compactor, on or about 2/8/17:  Stairway and stairway platform outside exit door was covered with ice and snow.  b)  Warehouse, third floor, on or about 2/8/17:  Floor at ramp at Location BA314 was broken up and not flush with the ramp, creating a tripping hazard.  c)  Warehouse, third floor, on or about 2/8/17:  Metal flooring was bent up in two areas near the elevator, creating a tripping hazard.  Abatement certification must be submitted for these items.
