29 CFR 1910.140(c)(11)(i): The employer must ensure that each horizontal lifeline is designed, installed, and used under the supervision of a qualified person; and is part of a complete personal fall arrest system that maintains a safety factor of at least two  In the finish shop the horizontal lifelines had not been designed and installed in a manner that ensured a safety factor of at least two. Employees were exposed to a fall hazard of up to approximately four and a half feet to the floor below.

In the finish shop the anchorages installed to support the horizontal lifelines had not been designed and installed in a manner that ensured a safety factor of at least two. Employees were exposed to a fall hazard of up to approximately four and a half feet to the floor below.
