29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. Production area - On or about February 10, 2017, The employer's energy control written program did not include specific procedures to control and lockout all energy sources on the Braner coil slitter and other operating equipment. Employees were thereby exposed to the hazards associated with the unexpected energization of machinery.

29 CFR 1910.179(g)(1)(v): Pendant control boxes shall be constructed to prevent electrical shock and shall be clearly marked for identification of functions. Production area - On or about February 10, 2017, employees were required to operate a Demag 25 ton crane and a Caldwell 1000 lb. crane. The employer did not ensure the identification of all functions were clearly marked on the pendent control boxes. Employees were thereby exposed to struck-by hazards.

29 CFR 1910.179(j)(2): Frequent inspection. The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: Production Bays - On or about February 10, 2017, overhead cranes operated by employees were not inspected at frequent intervals. Items not inspected included, but not limited to, functional operational mechanisms, hooks, hoist chains, and running rope. Employees were thereby exposed to struck-by hazards. The overhead cranes were as follows: 1. South Crane East Bay - Demag 25 ton (ser.#158272) overhead crane had not been inspected since 10/31/16. 2. West Bay North - P & H 10 ton (ser.#CE26644) overhead crane had not been inspected since 10/31/16. 3. North Crane East Bay - Damag 25 ton (ser.# 94527) overhead crane had not been inspected since 10/31/16.

Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected. For hooks with cracks or having more than 15 percent in excess of normal throat opening or more than 10�° twist from the plane of the unbent hook refer to paragraph (l)(3)(iii)(a) of this section. 29 CFR 1910.179(m)(1): Running ropes. A thorough inspection of all ropes shall be made at least once a month and a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier for the ropes which were inspected shall be kept on file where readily available to appointed personnel. Any deterioration, resulting in appreciable loss of original strength, shall be carefully observed and determination made as to whether further use of the rope would constitute a safety hazard. Some of the conditions that could result in an appreciable loss of strength are the following: Warehouse - On or about February 10, 2017, employees were required to operate overhead cranes. The employer did not ensure that inspections of the running ropes on each crane was performed on a monthly basis with certification records.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. Production area - On or about February 10, 2017, employees operating the Braner coil slitter were allowed to step iinside the frame area of the slitter to lift up sheets of steel that measured 48 inches by 72 inches. One or more methods of machine guarding was not provided to protect the employees inside the machine area. Employees were thereby exposed to struck-by and crushed by hazards.
