Guardrail systems were not installed before the scaffold was released for use by employees other than erection/dismantling crews.    On 1/6/2017, Employees from Casual Consultants Associates were installing metal support beams to a steel structure of a commercial building located at 17481 North Outer Forty Road, Chesterfield, MO.   Employees were operating JLG scissor lifts which did not have the safety latch chain properly attached.  Guardrail systems shall be installed along all open sides and ends of platforms.

The employee was working from a JLG scissor lift approximately fifteen feet above the ground.  The employee was standing on the mid-rails of a JLG scissor lift, in an attempt to gain additional height to perform his work.
