29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Location - 600 Main Street, Winchester MA:  On or about 02/01/2017 employees were using a homemade constructed ladder that did not extend three feet above the upper landing surface to gain access to the roof.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Location - 600 Main Street, Winchester, MA:  On or about 02/01/2017 employees were working on the roof withoutfall protection.  The Jose Caguana, dba JGC Construction Inc was previously cited for a violation of  an equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1062013, citation number 001, item number 003 and was affirmed as a final order on 02/24/2016, with respect to a workplace located at Lot 63, Mohawk path, Holliston, MA.
